Centralized Enrollment

This office is a one-stop location for your entire enrollment needs. New students in Kindergarten through Grade 12 will be enrolled at this location. You may pick up an Enrollment Packet at this office, or you may access Enrollment Forms by clinking on the links to the right.
What to bring with you to enroll a student:
Completed Enrollment Packet and Photo ID
Include the student’s social security card
Name and Address of previous school attended
Fax number is helpful if you have it accessible
Birth Certificate
Required for all students entering an Ohio school for the first time * if a birth certificate is not required for proof of custody, a copy from a previous Ohio school will be accepted
Proof of Custody
Birth certificate, court custody papers, or court filing
Proof of Residency
Current mortgage, tax bill, commercial lease, or residency affidavit with owners' proof attached and business mail in parents' name at that address.
Health Records
Records received from the previous school will be reviewed to keep within the guidelines for school enrollment.
Copy of the most recent IEP, MFE or 504 Plan
If applicable
The Centralized Enrollment Office is located at the Northwest Educational Service Center, 3310 Compton Road.
