Immunizations

Dear Parent:

Ohio law mandates children have certain immunizations (shots) for school attendance. The new immunization requirements are included in this letter. Your current 6th or 11th grade student will need to show documentation of having received these varying doses of the meningitis vaccine by the time he or she returns to school in the fall. Due to the supply and demand, the vaccine maybe more difficult to obtain over the summer, please fulfill this requirement as soon as possible

  • 7th Grade Requirement: One dose of the MCV4 (Meningococcal), Tdap (Diptheria, Teatanus, Pertussis) booster vaccine
  • 12th Grade Requirement:  Two doses of the MCV 4 (Meningococcal) by the start of the 2018-2019 school year. Please keep in mind that the vaccine requires at least eight weeks between the first and second dose. If the first dose was administered on or after your child’s 16th birthday, a second dose is not required.

Verification of your child’s completed immunization record must be on file with Northwest Local Schools within 14 days of the first day of school.

If your child has already received the vaccine please submit a copy of the vaccination record from your healthcare provider/clinic where it was received.   It may be faxed to CHS at 513.741.5032

If your child has not already received the vaccine, please contact your child’s doctor/clinic to schedule an appointment.
