NWHS Vaccination Opportunity

Dear NWHS Parents/Guardians,

In collaboration with Hamilton County Public Health NWLSD is offering an opportunity for all students age 16 and older to receive the Pfizer Covid-19 vaccination.

Northwest High School students are scheduled to receive the vaccinations at their home school during school hours. In-person and remote learning students can take advantage of this opportunity. Parents do not need to be present on vaccination day. This event is not open to the public.

Location: Northwest High School Main Gym

1st Vaccine Date/Time: Tuesday, May 4, 2021, 1:00-3:00 p.m.
2nd Vaccine Date/Time: Tuesday, May 25, 2021, 1:00-3:00 p.m.

NWHS parents/guardians will need to sign the Parental Vaccine Consent and Screening Form and register their child. Registration needs to be completed online. If you have difficulty registering, please complete and sign the Parental Vaccine Consent and Screening Form and send it with your child on vaccination day.

PLEASE READ THE REQUIRED DOCUMENTS BELOW:

Hard copies of the Parental Vaccine Consent and Screening Form can also be picked up at the Health Office in each high school.

Pfizer Vaccine Emergency Use Authorization: Vaccine information; no signature is required

Northwest High School Registration Link: *Required; must be completed by parent/guardian. Students 18+ may complete this themselves.

Northwest High School Parental Vaccine Consent and Screening Form: *Required; must be completed by parent/guardian. Students 18+ may sign their own Consent Form.
