Emergency Medical Authorization UPDATES

Parents/Guardians – Please note that the district will not send a hard copy of the Emergency Medical Authorization (EMA) form home with students this school year.  Parents are required to update their child's EMA electronically through Registration Gateway. Simply go to our website at www.nwlsd.org and click on Registration Gateway, or visit your child’s school to complete the brief online form.

By law, all students must have an updated Emergency Medical Form on file with their school.  You have until August 31st to complete the registration process.
